As a doctor you should be honest to your patients. If your patients need to be treated more, please tell them. You should be so sensitive to the financial condition from the patients. If the patients can't pay more treatment, just tell their families to try to find the another solution to pay for more treatment. So your patients will not complain you as an expensive doctor. Don't make it so easy because they will tell others not to come to you. You will lose other patients because of this reason.
As a doctor, you have more posts to improve your skill. One of the fields that needs your doctor skill is a forensic pathologist. You can be a good detective doctor. You can help police to solve their problem but you are still in your line as a pathologist.
